Seeking a dedicated Manager of Care Management to lead a team of RNs and Social Workers in delivering exceptional patient care. This full-time leadership role is responsible for overseeing daily operations and clinical practices within the Care Management team.

Under the direction of the Director of Care Management, you will guide Case Managers and Care Management Coordinators to ensure seamless patient care throughout the continuum. Your leadership will empower Case Managers to advocate for patients, optimize health outcomes, enhance access to care, and ensure the appropriate use of resources.
